What personal information do we collect from the people that visit our website, blog or mobile app (PLT Client Hub)?
When ordering or registering on our site, as appropriate, you will be asked to enter your personal details on a form. We collect information from you when you register with PLT, place an order for a plan or merchandise, subscribe to a newsletter, respond to a survey, fill out a form or enter information on our site, communicate with us on social media or request support.
When signing up for a PLT Nutrition membership, you also provide us with your personal details such as full name, email address, phone number, mailing address, date of birth, gender and preferred pronouns, height, weight, and medical history, how you heard about us (which may include the names of family members or friends who referred you), and your occupation. When you sign up for membership, we also collect information that you provide about your goals, activity levels, dieting history and your experience with tracking food intake as this helps us gauge your history and match you with a PLT Nutrition coach that suits your objectives and experience. This information helps streamline the on-boarding process so that a matched coach is up-to-speed with your information and history and can begin working with you as soon as possible.
During your membership, we collect information that you provide through the services, which may include, but is not limited to, information about your objectives and goals, nutritional intake, weight, and body changes, as well as the communications through the services between you and your coach.
We do not review communications between members and coaches except in the event of a dispute, but we reserve the right to do so for purposes of improving the Services. Members may also provide and share progress photos and information, such as weight loss successes and body changes that reflect your progress

Public Information including User Generated Content
You may choose to disclose information about yourself in the course of contributing User Generated Content to us or through your use of our Site and/or Services including but not limited to online forums, user 'profiles for public view or in similar forums on our Site. Information that you disclose in any of these forums is unencrypted, public information, may be accessed or recorded by PLT Nutrition, and there is no expectation of privacy or confidentiality therein.

We are not responsible for the Personal Information you choose to make public in any of these forums.

If you post a video, image or photo on our Site for public view you should be aware that these may be viewed, collected, copied and / or used by other users without your knowledge or consent. We are not responsible for the videos, images or photos that you choose to submit to our Site. Please refer to our T &C's on this point for other guidelines about posting content on our sites.
California Privacy Rights
If you are a California resident (“consumer”), the California Consumer Privacy Act of 2018 (“CCPA”) grants you the right to request that we disclose to you, upon our receipt from you of a verifiable request, the following:

Accordingly, we will ask you to confirm your postal address and the name of the coach on your account and we may ask for additional information you previously provided to us that may be in our records, and we also may request a copy of your valid government-issued identification. If the request to exercise rights under the CCPA is initiated by a coach, we will ask you to confirm your postal address and date of birth.
You have the right not to receive discriminatory treatment for the exercise of the privacy rights conferred by the CCPA. However, nothing prohibits us from charging a consumer a different price or rate, or from providing a different level or quality of goods or services to the consumer, if that difference is reasonably related to the value provided to the consumer by the consumer’s data.
You may exercise any of the California Privacy Rights described in this section by sending such a request to us by email. Consistent with California law, you may designate an authorized agent to make a request on your behalf. In order to designate an authorized agent to make a request on your behalf, you must provide a valid power of attorney, a copy of your valid government-issued identification and your authorized agent’s valid government-issued identification.
