Lifeline Assistance Program

DOCOMO PACIFIC's LifeLine/Link-Up Assistance Program is the Low-Income Program of the Universal Service Fund and is designed to benefit residential subscribers who are receiving assistance through one or more government benefit programs.

  1. Check to see if you or someone in your household qualifies for the Lifeline discount.

  2. Apply - Receive the fastest eligibility decision by applying online with the Lifeline National Verifier.

  3. Contact Us - Once your Lifeline eligibility has been approved by the Lifeline National Verifier, schedule an appointment with DOCOMO PACIFIC via the Skiplino App here.

DOCOMO PACIFIC's Lifeline Assistance Program is the Low Income Program of the Universal Service Fund and is designed to benefit residential subscribers who are receiving assistance through one or more of the following programs:

  • SNAP/Food Stamps
  • Medicaid
  • Supplimental Security Income (SSI)
  • Federal Public Housing Assistance
  • Veterans Pension or Survivors Pension Benefit; or
  • Total household income at or below 135% of Federal Poverty Guidelines

To prove a consumer’s household income is 135% or less than the federal poverty guidelines, consumers must submit one or more of the following documents*:

  • The prior year’s state, federal, or Tribal tax return
  • Current income statement from an employer or paycheck stub
  • A Social Security statement of benefits
  • A Veterans Administration statement of benefits
  • A retirement/pension statement of benefits
  • Unemployment/workers’ compensation statement of benefit
  • Federal or Tribal notice letter of participation in General Assistance
  • Divorce decree, child support award, or other official document containing income information

*If the consumer presents documentation of income that does not cover a full year, such as current pay stubs, the consumer must present the same type of documentation covering three consecutive months within the previous 12 months.

To prove participation in a Lifeline qualifying program, consumers must submit a document that, at a minimum, includes:

  • The consumer’s name, or the name of the consumer’s benefit qualifying person (BQP);
  • The name of the Lifeline-qualifying program, such as SNAP;
  • The government or Tribal program administrator or the managed care organization (MCO) that issued the document; and
  • An issue date within the last 12 months or a future expiration date that aligns with the benefit period.

Universal Service Fund - Low Income Program

The component of the Low Income Programs:

  • Lifeline - Reduces monthly charges for basic telephone service.

Eligibility Requirements for Residential Subscribers:

In order to meet and qualify for these rates, you must obtain a 'Certification of Eligibility' from a Life Line qualifying agency or program, for example: Department of Public Health & Social Services (DPH&SS), Social Security Office, Guam Department of Education National Lunch Program, the Guam Housing and Urban Renewal Authority (GHURA), etc.

In addition, the residential subscriber receiving the assistance must be the named participant of the qualifying program. One discount is allowed per household, limited to one service. Lifeline is a government benefit plan and is not transferable. Other conditions may apply.

You qualify for Lifeline if your income is 135% or less than the Federal Poverty Guidelines. Please see chart below.
