The premise behind patient portals (and patient engagement) is that IF we provide patients (or anyone) with enough health information, we can "empower" them to change their health behavior. There is a problems with this "belief". Cognitive psychologists and others across a variety of fields (including healthcare and education) have consistently demonstrated that "giving people more information alone "IS NOT ENOUGH" to change their behavior. Changing health-related behaviors, which are often life-long habits, and which are embedded in the social, physical, and cultural context in which people live their lives, require more than just information.

I think portals are great, but there is a lot of disparity in portal use. At one ethnic church teaching health promotion, only one lady had heard of the patient portal and I have about five. Most of the people had tech in hand even though they were primarily seniors. There is bias happening in not offering patient portals to everyone. That said, I think there is untapped use for the portal...we should be running algorithms and pushing health preventative screenings out. There has to be education on both the patient side and Provider side. Patients are often more tech savvy than we give them credit. The most recent example was during the pandemic and virtual visits. Just weeks before that, Providers and Organizations were convinced patients would not like or be able to figure out video visits and e-visits. Now, it's the preferred method for some. Having educational materials and knowledge pop-ups to show Providers what is capable and they will help progress your digital agenda at your Organization.

I definitely think there are disparities in technology, but I also think Health Organizations haven't fully appreciated the value of digital health. The cost of decreasing ED visits by providing patients with the ability to self monitor via a patient portal and intervene before it requires a visit to the ED is substantial. But, the idea of providing a device to a patient seems ridiculous. I am simplifying tremendously here, the process is more complex than this. If you can imagine though, the purchasing power of a huge health organization to have low cost devices, provide network data for patients to log into portals, and/or monitoring devices that integrate with your EHR, the savings in Healthcare would outshadow the cost.

I would like to see interoperability of portals so that regardless of who my providers are they can share information. When will this happen. I should not have 5 portals to check.....no doctor is going to do that....so really the portals are not helpful. For the current generation of patients the "horse has left the barn" when it comes to patient portals. Sure they have tremendous potential ... but you only get one chance to make a good first impression. I fear that too many of us were so unimpressed with our "first impressions" of patient portals (there is no there there) that it will be sometime before healthcare can convince them to "come back".
