Quality and safety challenges affecting NHS maternity services continue to make headlines, despite multiple improvement efforts over a long period. We studied large-scale improvement programmes in English maternity services over a period of 13 years. Many programmes have no publicly available evaluation report. Among those that have been evaluated, transparency of reporting and evaluation is often lacking, and significant areas of concern regarding programme quality and design are evident. This study suggests that large-scale improvement programmes would themselves benefit from improvement. Better design, delivery, evaluation and reporting would help maximise their impact, ensure accountability and better gather learning to improve care for patients. This! This is a really important read for all boards, very relevant beyond just maternity improvement programmes. This is relevant to so many elements of healthcare, especially boards’ accountability at all levels of the NHS, including Trusts, ICBs and NHSE, and will resonate with many staff, patients and patient groups who question why initiatives for quality and safety improvement don’t work and significant failings continue. Underpinning this are failures in integrated governance, assurance and risk management, rushing to the next fire and not ensuring change, including whether benefits were realised and the investment of resource (££, human, time) was evaluated, and, if found to reduce risk and improve quality and safety, embedded and built upon, rather than starting something new and carrying on the same loop again. Let’s break the cycle!!

This is great information to have on hand since the NC maternal mortality rate is rising faster than the national rate reported by the CDC in 2023. The U.S. maternal mortality rate in 2021 was 32.9 death per 100,000 live births, according to the CDC, up from 23.8 per 100,000 in 2020 and 20.1 per 100,000 in 20 19. NC’s maternal mortality rate doubled over those years, to 44 deaths per 100,000 births in 2021, up from 22 deaths per 100,000 in 2019.

The recent tragic case of a nurse's unthinkable actions has ignited discussions on patient safety. While it's crucial to address this horrifying incident, the long term solution lies in tackling broader systemic issues. Charlotte Augst, PPL Senior Advisor, discusses that alongside the horror we might feel about individual acts, we need also to focus on recurring patterns endangering patient safety: 1. Maternity and Neonatal Care: We must confront the unacceptable harm faced by mothers and babies due to under-resourced and poorly managed services, particularly impacting marginalised communities. 2. Listening to Patients: Learning from past mistakes, we should prioritise patient experiences as valuable sources of insight and act on their concerns. 3. Clinician-Manager Collaboration: Bridging the gap between clinicians and managers is essential for comprehensive care. Collaborative efforts should be promoted. 4. Recognising Bias: Acknowledging potential biases in assessing individuals can prevent overlooking warning signs.
