About The Position
The contractor shall perform the services below under the general direction and supervision of the Prince George's County Department of Social Services in the Children and Family Services Division (CAFS) in the Adult Services Division.
This position provides case management and crisis intervention to seniors aged 65 and over.
Examples of Work
What You'll Do
Conduct in-person home visits to interview clients and assess their stability, health, and socioeconomic needs.
Provide services to prevent abuse, neglect, and exploitation of vulnerable seniors.
Provide budget-dependent, gap-filling funds to help maintain the least restrictive environment.
Maintain weekly and monthly stats and update spreadsheets accordingly.
Coordinate referrals through community outreach
Maintain confidential filing of case records.
Review and process service funding plans
Write reports to evaluate the continued need for services and the ability to remain within the community.
Coordinate and collaborate with the Department of Aging and interdisciplinary teams to ensure the best delivery of service provisions.

Example
Bachelor’s degree in social work or human services
At least 2 years working with vulnerable children, disabled adults, or seniors.
In lieu of a degree, 7+ years working with vulnerable children, disabled adults, or seniors.
An equivalent combination of education, training, and experience will be considered.

Preferred Qualifications
Experience which includes coordination of services/activities with disadvantaged individuals
