What You'll Be Doing
Wake County Health and Human Services is looking for a Nurse Family Partnership (NFP) Nurse Home Visitor (NHV) to join the team! The NHV is responsible for providing comprehensive community health nursing services to women and their families eligible for the NFP Program. The NHV is responsible for maintaining highest standards in clinical nursing practice and adherence to the NFP model, and to policies procedures, guidelines and standards of NFP and Wake County Health and Human Services. The NHV reports to the NFP Nurse Supervisor.

Wake County Health and Human Services (WCHHS) is the consolidation of programs and services that include social services, public health, job search assistance, child support, and transportation. Our mission, in partnership with the community, is to facilitate full access to high quality and effective health and human services for Wake County residents. In addition to our numerous standard programs, Wake County Health and Human Services (WCHHS) is engaged in a number of special initiatives that are impacting services and programs throughout our entire agency. Hiring Range: $28.35-$38.27 commensurate with experience
Market Range: 28.35 - 48.20

Emergency Service Worker Statement
In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Essential Functions Include
- Monitors the client's progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are client focused
- Facilitates postpartum/prenatal depression screening/counseling with clients and makes referrals for ongoing services as appropriate
- Stays informed on current health care developments to provide safe, quality nurse home visiting services
- Completes all required NFP education
- Actively engages in skill building to meet all NFP Home Visitor competency requirements
- Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact nursing practice
- Develops and maintains community relationships to support client referrals
- Conducts community outreach of potential program clients
- Collaborates with all multidisciplinary team members and with a patient focus to implement the identified plan of care and facilitate and maximize client health care outcomes
- Consults and collaborates with the Nurse Supervisor and the NFP team during case conferences in the interest of providing optimal client services
- Maintains a positive relationship with physicians, nurses, other health providers, social workers, clients and their families to ensure that each client receives optimum care
- Interacts with other members of Nurse Family Partnership program team in ongoing program development implementation and evaluation

Required
The Basics (Required Education and Experience)
- Graduation from an accredited school of Nursing
- No specific experience required
- RN - Registered Nurse - North Carolina Board of Nursing or Compact state
- BPT - Bloodborne Pathogens Training
- CPR - Cardiac Pulmonary Resuscitation
- AED - Automated External Defibrillator
- Health Insurance Portability and Accountability Act (HIPAA)

- Bachelor's degree in nursing (BSN) strongly preferred
- Experience working with high risk pregnant patients
- Spanish language proficiency
- Ability to identify problems, determine possible solutions, and actively work to resolve the issues
- Ability to provide counseling on an individual basis to achieve individual service plan goals
- Ability to make decisions without regard to that personal prejudices, biases, and experiences
- Ability to use appropriate interpersonal skills and methods to reduce tension and resolve conflict
- Ability to actively participate as a member of a team to move toward the completion of goals
- Ability to demonstrate an understanding of applicable policies and procedures. Ability to maintain conditions that ensure a healthy and safe working environment
