Hackley Community Care (HCC) is seeking an LPN or Medical Assistant to provide clinical support services to youth and adults who suffer from chronic medical and mental health disorders. The patient population we serve at this location varies from patients with serious mental behavioral health challenges, developmental disabilities, chronic disease and/or substance use disorders. This person would offer clinical support services at our Integrated Health Clinic, located within HealthWest (formerly Muskegon County Community Mental Health). Our partnership with HealthWest allows us to offer primary care services, including dental services, to patients already receiving mental health services and collaboratively meet the unique health care needs of this patient population.

The LPN or Medical Assistant functions as a member of a multidisciplinary team to support the work of the healthcare professional medical team. Duties include, but not limited to coordinating care by gathering information, assisting in patient preparation, help administer, coordinate, educate and prepare immunizations, prepare medication refills, assist in social determinants of health, assist in pre-visit planning and may be responsible, at times, to assist in rooming patients, taking vitals, vision screenings and stocking exam rooms.

Job Qualifications and Special Skills include:

  • High school diploma or equivalent required.
  • Graduated from an accredited nursing or medical assistant training program.
  • Current active, in good standing, Medical Assistant certification (CMA/RMA/CCMA/NCMA) or LPN license.
  • CPR/Basic Life Support (BLS) certification required.
  • Ability to work with a diverse population.
  • Must possess professional and accurate written and oral communication skills.
  • Ability to work independently and within a team.
  • Proficient in computer and EHR software programs.
