Summary: The CPSP Case Manager individualizes a care plan that will provide the patient with integrated care addressing needs, promoting involvement and give health team members up-to-date information which optimizes quality of care.
Areas of Responsibility:
Initiate services and assessments for the CPSP orientation to include addressing and identifying patients in need of services, orientation referrals and/or intervention in the areas of perinatal services, nutrition, psychosocial and health education
Maintain complete client documentation that includes support services assessments, care plans, referrals, interventions, reassessments, and follow-up
Provide health education and educational materials to clients either as individuals or in groups
Provide information, encouragement, and education to patients about various insurance programs available for their status
Assist eligible patients with presumptive eligibility paperwork for Medi-Cal
Administrative duties include maintaining statistical and narrative reports related to the perinatal program, analysis of program process and needs, development of program policies and procedures
Surgery scheduling for OB/GYN
OB Referrals
Schedule GYN ultrasounds
Other duties as needed
Education/Experience:
Bachelor’s degree preferred
One year of prenatal care and education experience
Desired Skills:
Knowledge of local psychosocial education and nutrition resources
Extensive knowledge of various health services throughout the community
Knowledge of basic medical and perinatal terminology, procedures, and symptoms
Good written and verbal communication skills
Licenses and Certifications:
Certification of training of CPSP
Language Skills: Ability to effectively present information and respond to questions and requests from patients, co-workers, and others as necessary. Bilingual in English and Spanish, both written and verbal preferred.
