Care Coordinator - Remote | WFH

We are seeking a dedicated individual to join our team as a Field Care Coordinator. In this role, you will serve as the primary care manager for a panel of members with low-to-medium complexity medical/behavioral needs. Your focus will be on providing exceptional care coordination to support our members' medical, behavioral, and socioeconomic needs, thereby promoting appropriate utilization of services and enhancing the quality of care.

Position Details

Full-time position (40 hours/week) from Monday to Friday.

Flexible scheduling with 8-hour shifts between 8am to 5pm.

Occasional overtime may be required based on business needs.

Field-based position with a home office setup. You will work remotely when not in the field.

If you reside in the Southwest area of New Mexico or within commutable distance, remote work options are available.

Primary Responsibilities

Engage members face-to-face or via phone to conduct comprehensive needs assessments.

Develop and implement person-centered care plans addressing various needs.

Collaborate with internal care teams, providers, and community resources to execute care plans.

Provide education and coaching to empower members in self-managing their care.

Support discharge planning and manage care transitions following ER visits or admissions.

Advocate for members and ensure their needs are represented and supported.

Foster positive relationships and cultural sensitivity in care delivery.

Encourage self-advocacy and provide support for mental health and substance use treatment.

Educate members on community resources and benefits for enhanced self-sufficiency.

Foster social connections and support member engagement in activities and community networks.

Required Qualifications

18 years of age or older.

Relevant clinical experience and/or educational background.

Proficiency in MS Office applications.

Ability to transition between office and field locations.

Must reside in New Mexico and be willing to travel locally up to 50% of the time.

Reliable transportation and designated home workspace with high-speed internet access.

Preferred Qualifications

CCM certification.

Experience in community case management or managed care.

Background in team-based care.
