Job Summary

Under the supervision of the Wraparound Program Lead, the Child and Family Specialist (CFS) works with Wraparound Facilitator, Therapist, and Parent Partner and participates in the Child and Family Team to provide direct services to the child and family as identified in the Plan of Care. The CFS works with children and their families in their home/out-of-home placements and their respective communities. The CFS assists in determining clients’ need for community services and providing community resources in a timely manner. Additionally, the CFS completes all required documentation and paperwork in accordance with agency and county standards.

Essential Functions

  • Provide case management and advocacy services including evaluating and responding to identified needs for community services and resources, obtaining and facilitating client information, participating and providing feedback regarding service delivery and problem solving
  • With supervision serve as a liaison between client, agency and community agencies and organizations
  • Provide individualized treatment, individual rehabilitation, behavior modification, teaching and demonstrating activities, contribute to individual/group counseling and assist client in keeping appointments
  • With supervision help identify client needs and triggers to escalation (within a strength based model) and barriers and make recommendations to supervisors and treatment team
  • Report any concerns regarding client services and satisfaction to supervisor and appropriate team member
  • Provide oversight of clients from program admission in collaboration with the parents; develop individualized service plans based on assessment reports provided.
  • Conduct visits to homes, schools and other service providers in order to help client achieve their service plan goals.
  • Evaluate parent/child/family progress and participate in interdisciplinary team meetings.
  • Assist in the development of a comprehensive plan of care, ensuring strategies, interventions and services are family-centered, flexible and community-based.
  • Ensure collaboration, coordination, and partnership among all agencies and family involved in the implementation of the families’ plan
  • Assist family in the incorporation of informal supports, natural family helpers, and other community resources.
  • Provide 24-hour availability and crisis intervention on an as needed basis which may include weekends and evenings.
  • Maintain weekly accurate service time requirements, within expected deadlines.
  • Complete all required documentation, outcome measures, and paperwork in accordance with agency and county standards
  • Regular, active participation in administrative and clinical supervision and meetings.
  • Follow up with supervisor in a timely manner regarding significant issues.
  • Demonstrate ownership of work responsibilities by incorporating feedback, taking initiative, and interacting in a professional manner within assigned role and adhering to agency guidelines.
  • Actively participate in scheduled staff meetings, agency wide meetings, and other meetings as assigned
  • Sensitive and responsive to the cultural differences among the agency’s client population
  • Actively participate in quality improvement projects
  • Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse
  • Follow and adhere to all field safety guidelines.
  • Drive personal vehicle as necessary

The Child and Family Specialist will take on special projects or additional duties as assigned by the Program Lead.

Competency Requirements

  • Ability to read, write, speak and understand English
  • Bilingual (Spanish) preferred
  • Ability to work effectively as a member of the Wraparound team
  • Ability to successfully work with families of diverse ethnic and cultural backgrounds
  • Must have excellent oral and written communication skills
  • Must have excellent interpersonal skills
  • Must have strong organizational and planning skills
  • Knowledge of social services system and community resources
  • Knowledgeable in Wraparound philosophy, policy and process
  • Demonstrate skill in operating Microsoft Word, Outlook, Excel and other agency programs

Education And Experience

  • Bachelors degree in Social Work, Sociology, Psychology or related field
  • 1 year of experience working with seriously emotionally disturbed children and adolescents and their families preferred
  • Experience working in the Wraparound program preferred
  • Experience in crisis management preferred
  • Experience with DMH documentation preferred

Certifications/Licenses

  • Valid CA driver’s license
  • Valid automobile insurance

Additional Requirements

  • Tuberculosis clearance
  • Department of Justice/Fingerprint clearance
  • Satisfactory completion of health screening
  • Proof of legal eligibility to work in the United States
  • Satisfactory driving record

Physical Demands

While performing the duties of a Child and Family Specialist, the employee is frequently required to walk, drive, sit, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to stand and reach. The employee is rarely required to run, bend, kneel or lift objects weighing more than 30 pounds.

Work Environment

The work environment is indoor and outdoors. The noise level varies from quiet to loud. The work environment is community-based and requires conducting home visits
