Usually a "lessons learned" activity is prescribed in project delivery organisations, but greater value can be generated by changing the usual blame format into a learning opportunity: • ask people to list the good, bad and ugly outcomes of the project • don't waste time on the ugly. These are usually safety incidents or severe events THAT HAVE ALREADY BEEN DEALT WITH (hopefully) • don't spend too much time on the bad either. Usually these are mistakes from people that were trying too hard. They are embarrassed and have already implemented measures to prevent repeating them • look at the good - what went well. Find ways to repeat these outcomes.
