The first good practice is to start with the facts and the things observed. This notion of facts must be found in the question asked: What is the fact or facts that explain what we observe? From fact to fact, we can trace the root cause(s). An element of validation of a root cause lies in the countermeasure taken against it. If the countermeasure is a control, then the root cause has not been reached.
