Try to analyze where and how the mistake has occurred as if it was a human error that is OK as it happens or something else. Once it is done try to learn from it and find out the ways to rectify it or not to repeat the same in the future. Once the learning phase is completed now it is time to put those into action. Try to implement the steps to avoid making the same mistake. You always learn from mistakes so making it doesn't make you bad, but if you do not learn anything from it then you will. Lean into it! Don't avoid it, it's not the end of the world! Establish the "Why" first, then what were the circumstances leading up to the why. Was it lack of resources, lack of training, outdated training etc. If you find it was human error, that's ok, it happens. After you have corrected your mistake carry out a lessons learned internal audit and put what you learn into practice. Follow up on any training/coaching/mentoring that you feel may be beneficial. If you are struggling to find how it happened seek feedback. Sometimes an outside perspective is needed to set us on the right path
