Deixar muito claro que o erro é inevitável e é bem vindo! Isso é muito difícil em algumas culturas. Já trabalhei em times com cultura fortemente guiada a acertos, onde eu como líder do time tinha que mostrar frequentemente os meus próprios erros, rir destes erros diante do time e pedir ajuda para aprender e buscar outras alternativas. Outra abordagem interessante para os erros é estabelecer uma meta de erros por entrega - vai ser completamente contraintuitivo, e vai causar um choque inicialmente, mas vai ser libertador porque algumas pessoas precisarão errar para ajudar o time a atingir a meta de erros.
