One of the best tools I use is a Rolling Action Item List (RAIL) that I built in excel. The tool I built acts similar to One Note but in a more digestible format that can be shared. The key is identifying primary owners and supporters of action items, setting priority levels, groups and deadlines to be accountable to. This keeps you on task and team members up to speed on what open action items are out there, to mitigate issues in change management.
