Gather the facts and define the deviation. Then you will have more information on which of the above mentioned sections you need to focus on in order to uncover the cause. When there, you can engage with technical tools for further investigation and resolution, or workaround. Example of such processes would be Kepler-Tregoe's Situation Awareness and Troubleshooting processes.
