I have used this for both back and neck/head support and it works for all. While it is designed for lumbar support, it's soft enough that you can use it for other things just fine. It's fairly substantial so it can feel like a lot behind your back. I gave it 3/5 stars for "pain relief" simply because it hasn't really impacted that for me at all. I don't think my back pain is related to my posture though... so maybe don't read too much into that. If y back is feeling uncomfortable because of lack of support, this cushion fixes that. But I still have a sore back because I'm old...
